1)     What church are you attending?  How often do you attend: weekly, monthly, special occasions, never?  Who is the Youth Pastor at your church? 

2)     What extra-curricular activities are you involved in?  How often do you participate? (This can also include your hobbies and interests, eg. youth group, sport teams, job, clubs, writing, fishing, drawing, etc.) 

3)     Describe why you want to be a part of a Christian Leadership Training Program?

4)     What do you expect to learn from this program?

5)     Describe some of people or things that have the greatest influence in your life.  Why are they influential?
